Our pledge
Thrive Space Psychiatric Clinic is committed to protecting the privacy of your Protected Health Information ("PHI"). We are required by law to maintain the privacy of your PHI, provide you with this notice, and follow its terms.
How we may use and disclose your PHI
- Treatment: to provide and coordinate your care with other healthcare providers.
- Payment: to bill and collect payment from you, your insurer, or a third party.
- Healthcare operations: for quality improvement, training, audits, and other necessary administrative activities.
- As required by law: e.g., reporting abuse, public health activities, or in response to a valid court order.
Most other uses or disclosures of PHI — including marketing, sale of PHI, and most psychotherapy notes — require your written authorization. You may revoke authorization at any time in writing.
Your rights
- Inspect and request a copy of your medical record.
- Request a correction to your record.
- Request restrictions on certain uses or disclosures.
- Request confidential communications (e.g., contact by mail rather than phone).
- Receive an accounting of certain disclosures.
- Receive a paper copy of this notice.
- File a complaint with us or the U.S. Department of Health and Human Services if you believe your privacy rights have been violated. You will not be retaliated against for filing a complaint.
Changes to this notice
We may update this notice and apply the revised terms to all PHI we maintain. The latest version is always posted on this page.
